Monday Friday, 8:00 AM 5:00

PM Position Highlights:

Field-based within an assigned territory, providing daily patient home visits Expectation of 30 points per week (typically 35 patient visits each day)

Key Responsibilities:

Perform thorough OASIS assessments and develop personalized care plans Deliver skilled nursing interventions including wound care, medication administration, IV therapy, and comprehensive chronic disease management Monitor patient progress, document clinical observations, and communicate significant changes to the care team and physicians Provide clear education to patients and families about treatment plans, medications, and preventative care Ensure timely, accurate documentation using Home Care Home Base (HCHB) Work collaboratively with physical and occupational therapists, social workers, and home health aides for seamless care coordination Attend case conferences, staff meetings, and engage in continuing education opportunities Adhere to all regulatory requirements and standards for in-home patient care

Qualifications:

Active RN license in good standing in the state of practice Minimum one year of recent Home Health experience (field experience strongly preferred) Proficient with Home Care Home Base (HCHB) and OASIS documentation Valid drivers license, reliable transportation, and proof of auto insurance
