Step into a pivotal field-based nursing role delivering top-tier care to patients in their homes. This full-time, 13-week contract position offers a structured Monday through Friday schedule from 8:00 AM to 5:00 PM, with a caseload expectation of approximately 30 points per weektypically 3 to 5 patient visits per day. The role focuses on independently managing patient needs across a defined territory, utilizing both the OASIS assessment tool and the Home Care Home Base (HCHB) electronic documentation platform.

Key Responsibilities:

Perform comprehensive OASIS assessments and develop effective, individualized care plans for each patient. Deliver skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management. Monitor patient progress, promptly address any changes in condition, and communicate effectively with interdisciplinary care teams and physicians. Educate patients and families regarding disease management, treatment regimens, and health maintenance. Ensure meticulous, timely documentation within HCHB. Collaborate with physical therapists, occupational therapists, social workers, and home health aides to coordinate holistic patient care. Participate in regular case conferences, staff meetings, and ongoing learning opportunities. Uphold all regulatory and best practice standards for home health care.

Required Qualifications:

Active and unencumbered RN license in the state of practice. At least 1 year of recent home health nursing experience, with a preference for field-based backgrounds. Proficiency in HCHB electronic documentation. Direct experience with OASIS assessments. Dependable transportation, a valid drivers license, and current auto insurance.
