Home Health Registered Nurse (RN) - 36‑Hour Weekly Guarantee | No Weekends/On‑Call Job Overview This role is for a dedicated Registered Nurse (RN) Case Manager providing home‑based clinical care across Nash, Edgecombe, and Wilson Counties . The position offers a 36‑hour weekly guarantee , no weekend/on‑call requirements , and mileage reimbursement per contract company .
Pay Rate:
$50-$55/hr Responsibilities RN Case Management — Oversee a full patient caseload, complete assessments, coordinate care plans, and ensure continuity of care. Skilled Nursing Visits — Perform wound care, medication administration, disease‑management teaching, and safety evaluations. Care Coordination — Communicate with physicians, therapists, and other care team members to support patient outcomes. Documentation & Compliance — Maintain timely, accurate charting within agency and regulatory guidelines. Patient & Family Education — Teach medication adherence, symptom monitoring, and home safety strategies.

None Required Qualifications Active RN License — Verify RN licensure
Home Health Experience:
Required Strong assessment, communication, and care‑coordination skills Preferred Skills (Nice to Have) HCHB Experience Strong autonomy and time‑management abilities Comfort with mobile charting and EMR systems Dress Code Scrubs — no specific color required Medical, dental, and vision insurance are available to qualified candidates who meet eligibility requirements.
