H HOME Job Description:

Responsible to assess, plan, implement, evaluate, supervise and coordinate the home care of patients in accordance with professional standards of nursing and policies and procedures of Essentia Health. Provides direct patient care to patients requiring care for medical, diagnostic and emergency conditions.

Work Experience:

East Market:

Minimum of 1 year clinical nursing experience.

Education Qualifications:

Education as need for state licensure.

Licensure/Certification Qualifications:

Certification/Licensure Requirements:

Current nursing licensure in state (s) of employment. May be required to hold multiple state licensures, depending upon work location Basic Cardiac Life Support (BCLS) certification or ability to become certified within 1 month from date of hire

West Market:

Possession of a valid motor vehicle operator's license as verified through appropriate licensing agency Proof of current motor vehicle insurance

Detroit Lakes and Graceville:

Must meet the driving requirements and criteria acceptable to Essentia Health's insurer

Fargo Location Only:

Current registered nurse license in the state of North Dakota upon hire. Minnesota licensure required within 3 months of hire/transfer

EH Moose Lake:

Department 2065-2174000 : Possession of a valid motor vehicle operator's license as verified through appropriate licensing agency Proof of current motor vehicle insurance
