•LTC / SNF Responsibilities As a Registered Nurse (RN)

•Long-Term Care / Skilled Nursing , you will: Provide direct nursing care to residents in a Long-Term Care (LTC) / Skilled Nursing Facility (SNF) . Perform comprehensive nursing and resident assessments. Monitor residents and identify changes in condition. Administer medications and treatments according to physician orders. Monitor resident responses to medications and treatments. Develop, implement, and update individualized care plans. Document resident care accurately in the EMR . Communicate changes in resident condition to physicians and healthcare staff. Collaborate with LPNs, CNAs, physicians, and interdisciplinary healthcare teams . Assist with admissions, transfers, and discharges. Provide compassionate and resident-focused nursing care. Follow infection-control, safety, and facility policies. Maintain accurate clinical documentation and nursing records. Ensure compliance with applicable healthcare regulations and standards. Participate in weekend and holiday rotation as required. Required Qualifications Active and unrestricted Registered Nurse (RN) License . Current BLS Certification . Previous experience in LTC, SNF, Skilled Nursing, Long-Term Care, Nursing Home, Post-Acute Care, or Rehabilitation preferred . Strong nursing assessment and clinical skills. Strong communication and documentation skills. Ability to work independently and as part of a healthcare team. Ability to work in a fast-paced resident care environment. Experience with PointClickCare (PCC) or similar EMR preferred. Ability to work 12-hour night shifts . Availability for weekend and holiday rotation.
