Nebraska Methodist Health System is seeking a dedicated full-time RN Navigator for the Outpatient Behavioral Health Clinic in Fremont, committed to providing high-quality, personalized care to patients and their families.
Job Duties & Responsibilities:
Deliver comprehensive, age-appropriate nursing care through individualized assessments, interventions, and patient-centered service that promotes understanding, comfort, and positive outcomes. Conduct thorough initial and ongoing patient assessments, incorporating input from patients and families to identify needs, prioritize care, and support clinical decision-making. Develop, implement, evaluate, and revise individualized plans of care in collaboration with providers and interdisciplinary team members, ensuring continuity of care and appropriate referrals. Provide patient and family education regarding diagnoses, treatment options, prevention, screening, survivorship, and self-management strategies to support informed healthcare decisions. Coordinate and navigate care by collaborating with physicians, nurses, allied health professionals, and community resources to optimize treatment outcomes and patient support. Perform nursing procedures and technical skills safely and independently while adhering to evidence-based practice, organizational policies, regulatory standards, and quality improvement initiatives. Promote patient safety, risk reduction, and professional excellence through effective communication, resource stewardship, participation in treatment planning and clinical research initiatives, and adherence to the Nurse Practice Act and organizational standards.

Salary:
Pay starts at $38.00 an hour and may increase based on work history/experience.
Job Requirements:
Bachelors of Science in Nursing (BSN) from an accredited school of nursing required within 5 years of hire. Successful completion of annual mandatory education requirements. A minimum of 2 years nursing experience required. Previous ambulatory coordination experience required. Current valid Registered Nurse (RN) License, valid compact multi-state license, or a temporary permit while awaiting licensure required for work state. Current American Heart Association or American Red Cross Basic Life Support (BLS) strongly preferred at time of hire, required within 3 months of hire. Certification(s) in area of specialty preferred.
