Job Description

At Franciscan Healthcare, we believe exceptional care begins with compassionate caregivers. We are seeking a dedicated Registered Nurse (RN) to join our Home Health & Hospice team, providing personalized, high-quality care to patients in the comfort of their own homes. In this role, you will assess, plan, coordinate, implement, and evaluate individualized care for patients across the lifespan, including pediatric, adolescent, adult, and geriatric populations. As a vital member of our interdisciplinary team, you will build meaningful relationships with patients and families while promoting comfort, independence, dignity, and quality of life. Hours:
Minimum 35 hours/week to include Monday-Friday, days with an on-call rotation including nights, weekends, and holidays
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES
Assesses patient's total needs and evaluates home situation Arranges appropriate home care for patient's nursing needs Coordinates interdisciplinary team planning for each patient's goals; develops and completes a written care plan Participates in implementing planned changes and activities to improve nursing care Administers skilled nursing care, preventive and therapeutic treatments in the home setting Evaluates personal goals, revises, and formulates new objectives as necessary
QUALIFICATIONS
Education and/or Experience Graduate of an accredited school of nursing with a current licensure as a Registered Nurse in the State of Nebraska Bachelor of Science in Nursing preferred Minimum 1 year experience in home health, med/surg or critical care area preferred Valid driver's license Basic Life Support or ability to obtain within probationary period Other Competencies Interpersonal skills requiring courtesy and tact are necessary to communicate effectively Medium strength with lifting of 50 lbs. occasionally, 25lbs. frequently, and 10 lbs. constantly (Lifting over 35 lbs. requires assistive devices be used) Requires full range of body motion with frequent standing, walking, bending, stooping, and reaching Ability to properly lift position and transport patients (with assistance as defined) Ability to change from one duty to another frequently Analytical skills to organize and prioritize workload to meet patient needs Ability to stabilize unsteady patient when ambulating Ability to record information accurately and follow instructions exactly
