Job Description

Registered Nurse (RN) Case Manager LilyCare Hospice is a community-based hospice serving patients and families across southern New Hampshire. We are hiring a full-time RN Case Manager to join our founding clinical team. Benefits Medical insurance (PPO plan) Company-paid long-term disability coverage Paid time off and holidays Mileage reimbursement at the federal rate Direct access to clinical leadership — no layers between you and decisions Duties and Responsibilities Manage a caseload of hospice patients in private homes and facilities, completing assessments, individualized plans of care, and routine visits Provide skilled pain and symptom management in coordination with the attending physician and Hospice Medical Director Participate in IDG meetings and coordinate care across the interdisciplinary team, including hospice aides, social work, and chaplaincy Educate and support patients, families, and caregivers, and supervise hospice aide care per the plan of care Complete timely, accurate EMR documentation and participate in the on-call rotation Compensation and Schedule The RN Case Manager role pays $43.00-$50.00 per hour based on experience. Full-time, Monday-Friday day schedule with a rotating on-call assignment. Territory is southern New Hampshire, based out of Bedford, NH. Skills and Qualifications A current RN license in New Hampshire or a compact state One year of RN experience; hospice, home health, or case management experience preferred Current CPR certification A valid driver's license
