An opportunity is available for a skilled and compassionate Registered Nurse with home health expertise to deliver high-quality patient care in a field-based environment. This full-time, 13-week contract position focuses on providing comprehensive nursing services to patients in their homes, emphasizing continuity, clinical excellence, and effective communication.

Schedule:

Monday through Friday, 8:00 AM 5:00

PM Key Responsibilities:

Complete thorough OASIS assessments and create individualized care plans based on patient needs. Deliver skilled nursing interventions, such as wound care, IV therapy, medication administration, and chronic disease management. Track patient progress, identify significant changes in condition, and maintain prompt communication with physicians and interdisciplinary team members. Educate patients and their families about disease management, medications, and preventive health practices. Ensure timely and accurate documentation using Home Care Home Base (HCHB). Collaborate with therapists, social workers, and home health aides to ensure comprehensive patient care. Attend case conferences and staff meetings. Engage in professional development as needed. Adhere to clinical, regulatory, and documentation standards required by the agency and governing bodies.

Qualifications:

Active, unencumbered RN license in the state of practice. At least one year of recent home health (field-based) RN experience. Proficiency with Home Care Home Base (HCHB) and OASIS assessments. Ability to independently manage a caseload of 2532 points per week (typically 35 patient visits daily). Strong organizational, time management, and communication abilities. Reliable vehicle, valid drivers license, and current auto insurance. BSN and prior management of high caseloads preferred. Familiarity with Medicare and home health compliance is a plus.
