Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan members in their homes Evaluate patients prior and current medical and medication histories Educate individuals on their medical conditions and medications Pinpoint care gaps, addressing them with actionable recommendations Focus exclusively on comprehensive assessmentsno need to create treatment plans or prescribe medications

Qualifications:
