Key Responsibilities:

Conduct thorough annual and post-discharge health risk assessments for Medicare Advantage plan members in their homes Review and evaluate medical histories, chronic conditions, and current medications Educate patients and families on medical conditions and medication adherence Identify care gaps and recommend solutions to support ongoing health Documentation is essential; treatment planning and prescribing are not required

Qualifications:

Active Nurse Practitioner License in New Hampshire Current CPR (BLS) certification Board Certification via AANP or

ANCC MSN
