Full-Time What You'll Do As an RN Case Manager, you'll take ownership of a dedicated patient caseload while providing high-quality skilled nursing care in the home.
Your responsibilities will include:
Perform comprehensive nursing assessments and develop individualized plans of care. Provide skilled nursing visits and ongoing case management. Monitor patient progress and identify changes in condition. Coordinate care with physicians, therapists, caregivers, and other healthcare professionals. Educate patients and families on disease management, medications, treatments, and strategies for maintaining independence. Provide compassionate support to patients and families during important moments in their healthcare journey. Complete accurate and timely clinical documentation. Supervise and collaborate with members of the interdisciplinary care team. Advocate for patients and ensure continuity, quality, and safety of care.
