What You'll Do:

Conduct comprehensive, 45-minute home health risk assessments in patient residences, potentially as part of annual assessments or post-discharge visits Review and evaluate each individuals current and past medical history, medical conditions, and medication usage Educate patients on their health conditions and medications, promoting ongoing self-care and adherence Identify care gaps and provide recommendations to support improved health outcomes No responsibility for treatment plans or prescribing medicationsfocusing exclusively on assessment and education

Required Qualifications:

Active Nurse Practitioner license in New Hampshire Board Certification via AANP or ANCC Current CPR (BLS) certification MSN from an accredited nursing program At least six months of recent Nurse Practitioner experience (preferred) Commitment to patient-centered care and reliable, independent work
