Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage plan members Review and document prior and current medical history, existing conditions, and medication use Educate members about their health conditions and the purpose of their medications Identify care gaps and recommend actions to address those gaps No requirement to create formal treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner License in the State of NH Board Certification through AANP or ANCC Valid CPR certification (BLS) A minimum of six months experience as a Nurse Practitioner is preferred Master of Science in Nursing (MSN) from an accredited institution This role values expertise, efficiency, and compassionate care.
