Key Responsibilities:

Complete in-home annual risk assessments for a Medicare Advantage health plan Review and document members prior and current medical histories, conditions, and medication usage Deliver clear education on medical conditions and prescribed medications Identify and relay gaps in care with suggestions for improvement

Qualifications:

Active Nurse Practitioner license in New Hampshire Current CPR (BLS) certification Board Certification via AANP or ANCC At least 6 months of recent Nurse Practitioner experience preferred MSN from an accredited nursing program Compensation includes $80-$90 per completed evaluation, with additional mileage stipends and remuneration for administrative time.
