About Us Lift New Jersey is a DDD-approved provider agency supporting individuals with intellectual and developmental disabilities across New Jersey under the state's Medicaid waiver programs. We provide Community Based Supports, Individual Supports, Respite, and related services to individuals and families throughout the state. About the Role We're looking for a compassionate, detail-oriented Registered Nurse to join our team part-time, supporting individuals with medical acuity needs who receive services in their homes. This role is central to keeping our individuals safe and our agency compliant with New Jersey Division of Developmental Disabilities (DDD) requirements around medical acuity supports. What You'll Do Complete comprehensive initial nursing assessments for individuals with a medical acuity designation prior to the start of services Develop and maintain an individualized nursing plan of care for each individual, identifying nursing care needs and the services/supports required Update nursing assessments and plans of care at least annually, or sooner if an individual's needs change Train and support Direct Support Professionals (DSPs) on medical procedures and protocols specific to each individual's care plan Maintain accurate, DDD-compliant documentation for all nursing assessments and plans of care, available for review upon request Coordinate with Support Coordinators, care teams, and families to ensure medical needs are reflected in the Individual Service Plan (ISP) Support the agency's Attestation for Providers Rendering Supports to Persons with Medical Acuity by ensuring clinical practices align with DDD standards Respond to changes in an individual's health status and adjust care plans and staff guidance accordingly Participate in incident reviews or health-related documentation as needed Qualifications Active, unencumbered Registered Nurse (RN) license in the State of New Jersey (per

11-23) Experience working with individuals with intellectual/developmental disabilities strongly preferred Comfortable conducting home-based assessments and working independently in the field Strong documentation skills and comfort with compliance-driven, regulated care environments Reliable transportation for travel to individuals' homes across assigned NJ counties Excellent communication skills for coordinating with families, DSPs, and Support Coordinators Schedule & Compensation $50-$55/hour, based on experience Part-time W-2 position, approximately 5-10 hours/week to start, with potential to grow as our caseload of individuals with medical acuity expands Flexible scheduling based on individual assessment needs
