Qualifications:
Valid RN license, eligible to practice in NJ Minimum of 1 year recent experience in Rehabilitation or Long-Term Care settings BLS certification required Excellent clinical assessment, communication, and documentation skills Dedication to patient-centered care for individuals with complex and chronic health needs
Responsibilities:
Provide high-quality nursing care to patients in rehab and LTC environments Administer medications, monitor responses, and perform ongoing patient assessments Coordinate and implement individualized plans of care in collaboration with interdisciplinary teams Deliver patient and family education on care routines and rehabilitation goals Maintain accurate records while adhering to facility protocols and regulatory standards
