Primary Responsibilities:

Perform comprehensive home health risk assessments, approximately 45 minutes in length, for Medicare Advantage members Complete evaluations that may include annual health assessments and post-discharge visits Assess medical history, existing conditions, and medication regimens Deliver education on current health conditions and medication management Identify care gaps and suggest actionable recommendations No prescribing or treatment planning required

Qualifications:

Active Nurse Practitioner License in New Jersey Current BLS (CPR) certification Board Certification (AANP or ANCC) Minimum of 6 months experience as a Nurse Practitioner preferred MSN from an accredited school of nursing
