Central New Jersey Schedule:
Monday-Thursday, 5:00 PM-8:30 AM A well-established hospice care organization is seeking an experienced RN On-Call Nurse to provide compassionate, high-quality care to hospice patients and their families throughout North New Jersey. The RN On-Call Nurse is responsible for assessing patient needs, providing symptom management, coordinating care, and responding to patient and family needs during assigned on-call hours.
Responsibilities include:
Assume responsibility for assigned patient visits and the on-call schedule, including assessment, planning, implementation, and evaluation of nursing care. Provide hospice nursing care based on comprehensive assessments, with a focus on pain control and symptom management. Communicate with attending physicians, hospice team members, facilities, and other agencies as needed to coordinate patient care. Maintain regular communication with the direct supervisor regarding patient needs and assignments. Complete accurate and timely documentation at the point of service in accordance with agency policies and applicable Medicare regulations. Administer medications in accordance with federal and state regulations and organizational policies. Maintain current and accurate patient records, including interventions, goals, outcomes, and changes in condition. Assess patients and collaborate with the Medical Director and interdisciplinary team regarding hospice eligibility and ongoing care needs. Complete initial and ongoing assessments of patients and families to determine care needs. Anticipate end-stage disease progression and related pain and symptom-management needs. Regularly reevaluate patients' nursing needs and update the plan of care as appropriate. Provide preventive and palliative nursing interventions. Counsel and educate patients and families regarding end-of-life care and needs. Complete comprehensive death documentation when appropriate. Notify physicians of significant changes in a patient's condition or needs. Collaborate with members of the interdisciplinary team to coordinate the patient's plan of care. Participate in on-call responsibilities in accordance with organizational policies. Coordinate necessary medical equipment, supplies, and other services for patients.
Qualifications:
Associate Degree in Nursing required. Current and unrestricted New Jersey RN license . Previous hospice or home health experience required . Strong clinical assessment and documentation skills. Ability to work independently and make appropriate clinical decisions in an on-call environment. Excellent communication and interpersonal skills. Valid driver's license and reliable transportation. Ability to travel locally throughout the assigned North New Jersey territory.
