In this vital role, you will provide comprehensive nursing services within a supportive and dynamic environment, ensuring each individual receives personalized attention, medical oversight, and compassionate care. Your skills will help promote wellness, safety, and independence for our adult participants. Responsibilities Conduct thorough member initial and quarterly assessments, including vital signs and medical history review to determine individual care needs. Monitor the member's progress in reaching goals. Collaborate with interdisciplinary teams to develop and implement personalized care. Document all patient interactions accurately in medical records, ensuring compliance with HIPAA regulations and state and facility policies. Administer medications when required. Assist with the activities of daily living skills. Coordinate discharge planning. Monitor infection control best practices. Familiarity with geriatric (Geriatrics) populations. Communicate with the member's primary care physician. This position does not require night time or weekend hours. Paid vacation, holidays and personal days.
