Registered Nurse (RN)
Case Manager Hospice Pay:

As a Hospice RN Case Manager, you'll guide patients and families through one of life's most important journeys — ensuring comfort, dignity, and exceptional care every step of the way. You'll work independently in homes and facilities while being fully supported by a collaborative, mission‑driven interdisciplinary team that respects your clinical judgment and values your voice. What You'll Do Manage and coordinate care for an assigned hospice caseload with professionalism and compassion Complete comprehensive initial assessments and develop individualized, patient‑centered care plans Provide skilled nursing care in homes, assisted living communities, and nursing facilities Monitor patient conditions and adjust care plans based on clinical needs Educate patients and families on symptom management, disease progression, and end‑of‑life care Collaborate closely with physicians, social workers, chaplains, aides, and the full interdisciplinary team Document all care accurately and promptly in the electronic medical record (EMR) Participate in IDT meetings and contribute to care planning discussions Offer emotional support, guidance, and reassurance to patients and families Conduct supervisory visits for Home Health Aides (HHAs) and Licensed Practical Nurses (LPNs) What You Bring Active RN license in New Jersey 1-2 years of nursing experience (hospice or home health preferred, not required) Strong assessment, critical thinking, and clinical decision‑making skills Ability to work independently while staying connected to a supportive team Excellent communication and interpersonal skills Reliable transportation and a valid driver's license Why Nurses Love Working With Us We believe hospice nurses deserve the same compassion and support they give to others.
