Responsibilities:

Complete initial client assessments Develop and update client care plans Supervise NJ Certified Home Health Aides (CHHAs) Conduct required supervisory visits Review caregiver documentation Communicate with clients and families Ensure quality of care and NJ compliance

Requirements:

Active New Jersey RN license in good standing Must meet NJ requirements to serve as a Home Care/Health Care Practitioner Supervisor BSN preferred Home care, community health, or public health nursing experience required Good communication and organizational skills Reliable transportation
