Pines Village is looking for a motivated and team-oriented Full TIme , RN, MDS Coordinator to join our team. As a member of our care team, the MDS Coordinator will need to work Full TIme in office Monday - Friday and be available for the nursing on call schedule once a month.

Responsibilities of MDS Coordinator:

Must have experience with

MDS/PDPM

process for Sub-Acute and LTC residents Must have experience operating Point Click Care for assessments and EMR Collaborate with facility s staff to select the correct assessment of our residents Must have good communication skills in order to work effectively and keep accurate, detailed records Ensure all MDS are in compliance with State and Federal regulatory requirements Must have experience and participate in the Triple Check process Participate in developing/updating/reviewing/completion of individualized Care Plans Interested and qualified?
