Key Responsibilities:

Conduct annual and post-discharge home health risk assessments for Medicare Advantage members Review medical histories and current conditions, including medication history and management Provide patient education on health conditions and prescribed medications Identify and document care gaps, offering appropriate recommendations to address them No responsibilities for creating treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in New Jersey Board Certification via AANP or ANCC Current CPR certification (BLS) MSN from an accredited nursing program At least 6 months of current NP experience is preferred This role offers a competitive rate of $80-$90 per completed evaluation and includes additional compensation for mileage as well as administrative time.
