A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Job Description
Duties include:
Upon entry into out-of-home placement, assists with enrollment of a child in a managed care entity and identification of a medical home (primary care provider), and informs the child, caregiver, and DCF /CP&P, as needed. Assesses a child's health care status and needs by retrieving a complete health history through obtaining and reviewing all medical records and interviewing past providers and caregivers; forwards all information to the medical home and other health providers as needed. Schedules an Initial Physical Health Evaluation, in consultation with the caregiver and DCF /CP&P, reviews the preliminary and final reports, and ensures that all recommendations for health care are completed. Develops a child health care plan in collaboration with the child's medical home, as well as involved specialists, caregivers, other community service providers, and when appropriate, DCF /CP&P. Links the child and caregiver with appropriate health resources and community services. Assures that the child is scheduled for and attends timely EPSDT visits and specialty appointments, reviews all reports, and ensures all necessary reports are forwarded to the medical home. Bachelor's Degree in Nursing or a BA / BS degree in a health-related field required; may substitute years of pediatric nursing experience for degree.
