Responsibilities:

Provide direct patient care, including administering medications, performing treatments, and providing wound care, as ordered by the physician.
Assessment and Intervention:
Conduct initial and ongoing assessments of residents' needs, developing and implementing individualized care plans to address identified needs.
Communication:
Collaborate with interdisciplinary teams, including physicians, therapists, and other healthcare professionals, to ensure seamless care transitions and effective patient outcomes.
Documentation:
Maintain accurate and timely records of resident care, progress, and any concerns.
Quality Improvement:
Participate in quality improvement initiatives to enhance patient care and satisfaction.
Support:
Assist with orientation and training of new staff members, supporting the growth and development of our team.
