Overview:
We are seeking a highly skilled, detail-oriented, and compassionate Licensed Practical Nurse (LPN) to join our team as a Pre-Admission Chart Nurse. In this role, you will be responsible for conducting comprehensive pre-admission assessments, reviewing and validating clinical documentation, and ensuring the accuracy and completeness of patient information prior to admission. You will play a critical role in promoting patient safety, supporting quality outcomes, and ensuring compliance with Centers for Medicare & Medicaid Services (CMS) and Accreditation Association for Ambulatory Health Care (AAAHC) standards. The ideal candidate demonstrates strong clinical judgment, exceptional communication skills, and the ability to collaborate effectively with physicians, patients, and interdisciplinary healthcare teams to facilitate a seamless admission process. Responsibilities include, but are not limited to: Review patient data for completeness and accuracy Verify patient demographics, medical history, allergies, current medications, and insurance information. Ensure all required physician orders, informed consents, and pre-admission documentation are completed. Review laboratory results, diagnostic tests, imaging reports, and electrocardiograms (ECGs) to confirm availability and completeness. Identify missing, incomplete, or inconsistent documentation and coordinate with physicians' office and clinical staff to obtain the required information. Contact patients to complete pre-admission assessments and verify health history as needed. Assess patients for readiness for admission or scheduled procedures using established clinical guidelines. Communicate abnormal findings or potential concerns to physicians, anesthesia providers, and other members of the healthcare team. Provide patients with pre-admission instructions, including medication, fasting, and procedural guidelines when appropriate. Ensure compliance with hospital policies, accreditation standards, and regulatory requirements. Maintain patient confidentiality and protect health information in accordance with HIPAA regulations.
