A Public or Community Health Nurse provides health care to community members and addresses community health issues ranging from infectious diseases to substance abuse, focusing on prevention and education as well as treatment. Works with a team of health care professionals; may work in a clinic, and travel within a community or between different communities. May serve as the primary health care provider in underserved communities.
The City of Rahway Health Department is seeking a full-time New Jersey-licensed Registered Nurse to serve as a Graduate Nurse in Public Health. This position works within the community to support population health and provide preventive health services. The Graduate Nurse Public Health works under the supervision of a licensed Public Health Nurse and the Health Officer and assists with a variety of public health nursing programs and services. Essential Duties and Responsibilities The Graduate Nurse Public Health will: Monitor and assess communicable diseases using the statewide communicable disease reporting and surveillance system. Evaluate health trends, risk factors, and health concerns affecting population groups and assist in identifying priorities for targeted interventions. Participate in nursing and immunization clinics under the supervision of a licensed Public Health Nurse and Health Officer. Conduct public health screenings at various community locations. Provide appropriate referrals to local agencies and community resources based on individual and family needs. Communicate with patients and their families regarding care, recovery, and available health resources. Educate individuals and families on disease prevention and healthy lifestyle practices. Provide education and guidance to parents of young children regarding public health and immunizations. Provide educational resources on home nursing, maternity and pediatric care, and basic individual and family well-being. Maintain vaccine and medical supply inventories. Complete and maintain required documentation, including patient records, nursing logs, and other reports. Ensure compliance with applicable State and Federal medical safety, legal, and regulatory requirements. Maintain patient confidentiality in accordance with applicable healthcare regulations. Participate in community health activities and other Health Department programs as assigned. Perform additional duties and responsibilities as assigned by the Health Officer or designee.
Minimum Qualifications Education and Licensure:
Graduation from an accredited college or university with an Associate's degree in Nursing. Valid New Jersey Registered Nurse (RN) license. Valid New Jersey driver's license.
Experience:
One year of nursing experience in a public health setting; or Nursing experience obtained while working under the close supervision of a licensed Public Health Nurse. Bilingual ability is a plus. Additional Requirements Successful completion of a pre-employment drug screening and background check. Ability to maintain patient confidentiality and comply with applicable healthcare laws and regulations. Ability to work effectively with individuals, families, healthcare providers, community organizations, and other public health partners. Knowledge, Skills, and Abilities The successful candidate should demonstrate: Knowledge of public health nursing principles and practices. Knowledge of communicable disease prevention, reporting, and surveillance. Ability to communicate effectively with patients, families, and community members. Ability to provide health education to individuals and groups. Ability to maintain accurate and complete medical and administrative records. Ability to work independently while also functioning effectively as part of a public health team. Ability to establish and maintain effective working relationships with community agencies and healthcare providers. Commitment to providing culturally appropriate, compassionate, and accessible public health services. Work Environment The position involves both office-based and community-based work. The employee may conduct screenings, participate in clinics, provide education, and perform other public health activities at various locations throughout the community.
