Registered Nurse In-Home Assessments (Per-Diem)

Job Overview Caring Connections Home Care is seeking a compassionate and reliable Registered Nurse (RN) to join our team on a per diem basis in New Jersey. This position focuses on conducting in-home assessments for clients throughout Bergen and Passaic Counties . It's a meaningful opportunity to make a real impact by ensuring high-quality care for clients in the comfort of their homes. While prior experience is preferred, we are open to considering motivated new RN graduates who are eager to learn and grow in a supportive environment. Responsibilities include conducting in-home assessments, developing care plans, communicating findings with the Director of Patient Services (DPS), completing timely and accurate documentation, and providing ongoing supervision of Home Health Aides (HHAs) to ensure quality of care and adherence to care plans. Building strong, professional relationships with clients, families, and HHAs is essential to promoting trust, communication, and continuity of care. Applicants must hold an active Registered Nurse (RN) license in New Jersey , have a personal vehicle for travel within Bergen and Passaic Counties, possess solid computer skills (all documentation is electronic), and demonstrate strong communication, organizational, and time-management abilities. Flexibility, professionalism, and a genuine passion for working with elderly clients in their homes are key qualities for this role. Experience in home care, conducting assessments, or supervising HHAs is preferred but not required.
