Responsibilities What You'll Do:
Get the
Best Training:
Start with a two-week clinical orientation to learn everything you need to succeed
  • from key procedures to our EMR system, safety protocols, and compassionate care.
Be the
Expert:
Assess patients with complex health needs, develop personalized care plans, and ensure the best outcomes. Make a
Connection:
Educate and guide patients and families through their healthcare journey
  • helping them understand their conditions and the path to better health.
Collaborate for
Success:
Work closely with a team of VNA clinicians and external providers to ensure seamless care and optimal results for every patient. Document with
Purpose:
Record and track changes in patient conditions, updating care plans to reflect their progress and needs. Qualifications What We're Looking for in
You:
We're searching for a nurse who's not only skilled but also compassionate and motivated to help patients achieve their health goals. If you're ready to make a meaningful impact on people's lives, we want to hear from you. Registered Nurse (RN) with a Bachelor's Degree in Nursing (preferred). 1+ year of clinical experience as a registered nurse (bonus points if you have experience in homecare!). Strong verbal and written communication skills
  • you'll be guiding and educating patients and families. Proficient with technology
  • navigating EMR systems and using digital tools to enhance patient care. A reliable car and valid driver's license
  • you'll be traveling to patient homes, so your own transportation is key.
Physical Demands/Working Conditions:
Flexibility to work both indoors and outdoors while using clinical equipment. Comfortable using a phone, computer, and other digital tools to stay connected with your team and patients.
