The Center for Hope Hospice and Palliative Care is a unique Union County-based nonprofit organization seeking a Nurse Practitioner (NP) for our palliative care program. The NP will provide palliative care services in both the community and hospital settings, including home visits throughout Union County and surrounding areas and inpatient palliative care consultations at local hospitals. The NP works collaboratively within a multidisciplinary team and is responsible for providing comprehensive and age-appropriate care to both well and/or ill patients. The NP will provide consultative assessment and comprehensive evaluation of seriously ill patients and their families, addressing symptom management, therapeutic understanding, and prognostic awareness. The NP will collaborate and coordinate care along with the interdisciplinary care team to ensure the plan of care is in line with and supports the patient's goals of care and physical and psychosocial needs. The NP will be responsible for documentation of consultations and follow-up reassessments in the electronic medical record, along with prescriptive recommendations and/or prescriptions.

Minimum Qualifications:

Master's degree in Nursing. Current Registered Nurse license and Nurse Practitioner license. Current CDS and DEA. Current CPR certification. Minimum of 3 years of NP experience. Palliative care experience preferred. This is a per-diem position, and compensation will be based on Relative Value Units for completed appointments. A minimum of 3 days per week is preferred.
