Role Overview:

Conduct thorough annual home health risk assessments (approximately 45 minutes per visit) Evaluate patients prior/current medical histories, medical conditions, and medication use Educate members about their medical issues and medications in a home setting Identify any care gaps and provide clear, actionable recommendations to address them Visits may entail annual wellness or post-discharge assessments No requirement to develop treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in New Jersey Board Certification via AANP or ANCC Current CPR certification (BLS) Master of Science in Nursing (MSN) from an accredited institution Minimum 6 months recent Nurse Practitioner experience preferred
