Responsibilities:

Provide certified home health care services to clients in their homes or in our community-based settings Conduct initial assessments and develop care plans tailored to each client's needs Deliver skilled nursing services, including wound care, medication management, and IV therapy Collaborate with other healthcare professionals to ensure seamless care transitions Document client progress, identify areas for improvement, and implement changes as needed

Requirements:

Certified Home Health Nurse (ASAP certification preferred) Active NJ State licensure 0 years of experience in home health care (training provided) Strong communication and interpersonal skills Ability to work independently and as part of a team Compassionate and patient-focused approach to care
