Community Care Partners is seeking a bilingual Licensed Practical Nurse (LPN) to serve as a Case Manager supporting adults with intellectual and developmental disabilities throughout New Jersey. This role combines clinical oversight, care coordination, and case management to help individuals access services, maintain their health and well-being, and achieve their personal goals while living in the community. Responsibilities Coordinate and provide care that is safe, timely, effective, efficient, equitable, and person-centered Manage assigned caseload and ensure individuals receive appropriate services and supports Develop, implement, and monitor individualized plans of care Conduct nursing assessments and monitor individuals' health status and medical needs Collaborate with physicians, specialists, families, support coordinators, and interdisciplinary team members Attend ISP meetings and provide clinical input as needed Supervise assigned Direct Support Professionals (DSPs) in the field and provide guidance regarding health-related concerns Maintain a minimum of monthly contact with individuals on assigned caseloads and conduct quarterly visits with individuals and families Review documentation and activity logs to ensure accuracy and compliance Help individuals and families navigate healthcare services and community resources Develop effective working relationships with Support Coordination agencies and healthcare providers Adhere to agency policies, procedures, and DDD requirements Attend relevant professional development and training opportunities The employee will be responsible for conducting visits at clients' homes and in the community. Weekly mileage reimbursement is provided. Qualifications & Skills Current New Jersey Licensed Practical Nurse (LPN) license in good standing Experience working with individuals with intellectual and developmental disabilities (I/DD) preferred Knowledge of DDD programs and requirements strongly preferred Previous case management or care coordination experience preferred Supervisory experience is a plus Strong clinical assessment and documentation skills Excellent communication, organizational, and time management skills Ability to prioritize tasks, solve problems, and manage multiple responsibilities Valid driver's license and reliable transportation required English and Spanish fluency required Join our team and make a meaningful impact in the lives of individuals and families across New Jersey!
