Job Description

CVS Health is seeking a Case Manager Registered Nurse to coordinate comprehensive, patient-centered care for members across the continuum. In this role, you will perform telephonic and virtual assessments, develop individualized care plans, and close gaps in care using evidence-based guidelines. You will collaborate with physicians, pharmacists, and social workers, address social determinants of health, and support safe transitions of care.
