• Perform comprehensive assessments for new admissions and acute issues in both subacute and long-term care patients
• Prevent avoidable hospital transfers through early evaluation and timely intervention
• Collaborate and communicate effectively with primary care providers and specialists
• Participate as an active member of the interdisciplinary care team Qualifications
• Previous RN experience required — New NP graduates are welcome to apply!
• Strong critical thinking and assessment skills
• Ability to thrive in a fast-paced environment
• Excellent written and verbal communication skills
