What they do
A Registered Nurse provides medical care and treatment, educates patients about their conditions, and provides emotional support to patients and families. Works in hospitals, schools, clinics or community health centers. Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.

Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). Observe and report any signs or symptoms indicative of changes in condition of the patient and family situation. Document such reports and observations in the clinical record and notify supervisor. Regularly re-evaluates patient nursing needs. Initiates the plan of care and makes necessary revisions as patient status and needs change. Uses health assessment data to determine nursing diagnosis. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the provider. Provides direct patient care as defined in the State Nurse Practice Act. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan of care. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Prepares clinical notes and updates the primary provider when necessary and at least every 60 days. Communicates with all relevant providers regarding the patient's needs and reports any changes in the patient's condition; obtains/receives orders as required. Communicates with community health related persons to coordinate the care plan. Performs other duties as assigned.Qualifications:
Graduate from an accredited registered nursing program and licensed in the state At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred) Home health experience preferred Possess and maintain valid CPR certification.
