Job Description

Position Summary The ACT (Assertive Community Treatment) Team Nurse provides direct nursing support to clients with severe and persistent mental illness enrolled in the ACT program. This part-time position ensures comprehensive, person-centered care by working collaboratively with a multidisciplinary team. The nurse is responsible for monitoring health status, medication safety, and coordinating medical services, all in alignment with the ACT model and LifeHouse standards. Key Responsibilities Clinical Care and Monitoring Conduct regular health assessments for ACT clients and respond to emerging medical or behavioral health concerns Monitor medication adherence, efficacy, and side effects, including those related to medication-assisted treatment Educate clients, their supports, and team members on health and wellness topics and psychiatric medications Coordination and Collaboration Collaborate with internal and external healthcare providers to support integrated care Link clients to appropriate medical providers and specialists, ensuring follow-up on all referrals and care recommendations Initiate and maintain relationships with outside service providers, human service agencies, and other relevant stakeholders Team Participation and Model Fidelity Actively participate in team meetings, individual supervision, and group supervision Contribute to quality improvement initiatives and ensure fidelity to the ACT model Assist in formal training processes and participate in ongoing professional development Documentation and Compliance Complete all documentation in a timely and accurate manner, consistent with LifeHouse policy and regulatory requirements Uphold all confidentiality standards and regulatory compliance including those from the New Mexico Regulation and Licensing Department, Medicare, the Commission on Accreditation of Rehabilitation Facilities (CARF), and
LifeHouse Position Requirements Minimum Qualifications:
Active Registered Nurse (RN) license in the State of New Mexico Strong knowledge of mental health and substance use treatment, including familiarity with psychiatric medications and medication-assisted treatment Ability to provide care using a trauma-informed and recovery-oriented approach Strong written and verbal communication skills Ability to work effectively in community, home, and office settings Capacity to prioritize and plan work in a fast-paced, team-based environment
Preferred Qualifications:
Experience working with individuals with behavioral health needs or severe and persistent mental illness Previous experience in a multidisciplinary treatment team or ACT model setting
