About the Role We are seeking an RN/MDS Coordinator to own the clinical assessment process at Laguna Rainbow Elder Care, a skilled nursing facility in Casa Blanca, New Mexico. This role is central to ensuring that every resident receives a care plan that accurately reflects their needs — and that the facility maintains the documentation integrity required to sustain its mission. Laguna Rainbow Corporation exists to serve elderly citizens residing on or near the Laguna Indian Reservation. As MDS Coordinator, you will lead the RAI/MDS process from admission through ongoing care planning, partnering across disciplines to ensure assessments are accurate, timely, and compliant — while honoring the dignity and cultural identity of the residents you serve. This is a role for a clinician who brings both assessment precision and genuine care for the elderly — someone who can manage the technical demands of MDS documentation without losing sight of the people those documents represent. What You'll Do Lead the full RAI/MDS process, including assessments at admission, quarterly, annually, and with any change in status Coordinate the Interdisciplinary Care Plan team across all departments to ensure timely, accurate completion of MDS forms, RAPs, and care plans Complete Medicaid abstracts within 72 hours of admission; manage concurrent reviews and reapplications on schedule Maintain a well-organized, accessible assessment and care plan documentation system Monitor the ICP process for compliance and flag gaps before they become financial or regulatory exposure Partner with the Director of Nursing, Controller, and Administrator on reporting and assigned projects Drive adoption of the electronic MDS/care plan system and support conversion to updated software as needed What We're Looking For Active RN license in the State of New Mexico — required 2+ years of geriatric nursing experience and/or MDS coordination — strongly preferred Working knowledge of RAI/MDS processes and CMS electronic reporting requirements Strong written and verbal communication skills; comfortable presenting to interdisciplinary teams Organized, self-directed, and able to manage competing deadlines without losing accuracy Genuine care and patience for working with elderly residents and their families Why This Role Matters Accurate MDS documentation isn't just a compliance requirement — it directly determines reimbursement for the residents in your care.
