Job Summary MediConnect Inc. is seeking a mission-driven Registered Nurse who wants to help build the next generation of care delivery in New Mexico. This is not a traditional bedside nursing role. This is an opportunity to work at the front edge of Mobile Integrated Health, Community Paramedicine, mobile care delivery, remote patient monitoring, and community-based care coordination for patients who need clinical support where they live, not only where healthcare facilities are located. At MediConnect, our clinical teams combine mobile visits, remote monitoring, care transitions, chronic disease support, social-needs navigation, and technology-enabled follow-up to help patients remain safer at home, avoid unnecessary emergency department utilization, and stay connected to the right level of care. We are looking for an RN who enjoys clinical problem-solving, patient education, interdisciplinary teamwork, and building practical systems that make healthcare more accessible. This role is ideal for an RN who wants more autonomy, more variety, and more direct community impact than a conventional facility-based position. The RN will serve as a clinical anchor for a mobile care team that may include LPNs, EMTs, community health workers, care coordinators, remote monitoring staff, payer partners, primary care providers, specialists, and EMS partners. The RN will help translate patient needs into actionable care plans, clear escalation pathways, and measurable outcomes. Why Join MediConnect? The RN is the primary nursing authority for determining whether member-reported symptoms, RPM alerts, home-visit findings, or SDOH issues require a nursing intervention, provider escalation, EMS/MIH involvement, or payer case-management notification. This position is central to the program's New Mexico compliance structure because the RN owns nursing assessment, evaluation, judgment, and delegation decisions. New Mexico nursing standards require the nurse to maintain competence, function within the legal boundaries of nursing practice, and delegate only to persons prepared, qualified, licensed, or certified to perform the assigned activity. The RN must therefore ensure that LVN/LPNs, CHWs, RPM support staff, and coordinators receive only those tasks that match their role, competency, and supervision pathway. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately, including IV infusions and botulinum toxin treatments, following strict aseptic techniques and safety protocols. Assist with diagnostic procedures. Coordinate patient care plans, discharge planning, and case management for diverse populations including geriatrics, pediatrics, behavioral health, and individuals with developmental disabilities. Ensure compliance with HIPAA regulations by maintaining accurate medical documentation and medical records while safeguarding patient confidentiality. Collaborate with multidisciplinary teams to deliver holistic care in settings such as hospice & palliative medicine, occupational health clinics, or outpatient clinics. Requirements Valid RN license with current registration to practice in the relevant jurisdiction. Familiarity with EMR/EHR systems like Epic or Cerner; experience with
ICD-10/ICD-9
coding is highly desirable. Strong knowledge of medical terminology, physiology, anatomy, and diagnostic evaluation techniques. Ability to perform technical skills including IV insertion, tube feeding management, sterile processing, and aseptic technique adherence. Experience working with diverse patient populations. Excellent communication skills for patient assessment and health coaching; capable of triage in urgent care or emergency room settings. Additional certifications such as Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), or specialized training in trauma medicine or infectious disease care are advantageous.
