Job Responsibilities Coordinates home care services with clients and their families Review clinical components of care provided Ensure home care aides are supervised post-orientation in the home and that skills of the aide match the care needs of the client Visits new referrals for home care to evaluate new clients and start a plan of care Elevate any concerns in the home to the Home Care Director Schedule ongoing weekly/monthly visits with clients and communicate any changes to care team Submit documentation on a timely basis Key Qualifications RN degree One year experience in a health care setting, home care preferred. Solid understanding of home care and field staff disciplines Staffing flexibility Solid technology skills Demonstrated ability to build strong relationships High impact communication skills; both written and verbal Strategic thinker with strong attention to detail Self starter, able to prioritize, meet tight deadlines and manage time effectively Demonstrated ability to problem solve and implement solutions collaboratively Strong organizational skills
