! Creek View Kids , a division of Creek View Home Health , is currently hiring Registered Nurse (RN) for consistent - yet flexible - private duty nursing shifts in Pahrump, NV. As a member of our Home Health team, the RN will plan, organize, coordinate, and direct home health services (as needed) and provide direct patient care and treatment with emphasis to engage in autonomous patient health education. We strive to provide a consistent schedule to our patients as the bond built with their nurse plays an incredible role in fostering companionship, trust and personalized comprehensive medical care. Creek View Kids provides our clinicians vast options to provide the flexibility and consistency to match the work-life balance of our nurses while upholding our service commitments with our patients.- Visits are available daily Sunday- Saturday daytime, afternoon, evening and overnight
Responsibilities:
Communicate with patient and/or caregiver confirmation of scheduled visit while conducting pre-visit screening. Review and conduct established plan of care and follow the physician prescribed orders to include but not limited to:- Administration of prescribed medication, treatment, and therapies
- Gastrostomy tube feedings and care
- Tracheostomy care
- Mechanical ventilation monitoring
- Wound care Routinely conduct patient assessment(s), initiate appropriate preventive and rehabilitative nursing procedures and educate patient and families in care coordination Provide accurate and timely documentation via KanTime Home Healthcare Software Maintain proactive and timely communication via TigerConnect Opportunity to conduct start of care admissions, recertification assessments, and mentor
