Schedule:

Key Responsibilities:

Deliver comprehensive in-home nursing care and skilled interventions Accurately complete patient assessments and OASIS documentation within required timelines Collaborate closely with physicians and interdisciplinary care teams to implement individualized care plans Execute wound care, medication management, patient teaching, and ongoing disease monitoring Communicate significant changes and updates to the clinical team promptly

Required Qualifications:

Active RN license (unrestricted) At least one year of recent RN home health experience OASIS documentation experience Familiarity with HomeCare HomeBase is strongly preferred Consistent reliability and ability to manage daily, structured patient visit schedules Valid drivers license and reliable transportation
