Job Summary We are seeking a compassionate and dedicated Registered Nurse - Home Health to provide high-quality patient care in the comfort of their own homes. The ideal candidate will possess a strong background in various medical fields and demonstrate exceptional clinical skills. This role requires the ability to work independently while collaborating with a multidisciplinary team to ensure comprehensive patient care. Duties Conduct thorough patient assessments, including medical history and physical examinations. Develop and implement individualized care plans based on patient needs and preferences. Administer medications and treatments as prescribed, ensuring adherence to aseptic techniques. Provide education and support to patients and their families regarding health management and treatment options. Monitor patient progress and adjust care plans as necessary, documenting all changes accurately. Collaborate with healthcare professionals, including physicians and therapists, to coordinate care. Ensure compliance with Medicare / Medicaid / HIPAA regulations and laws Maintain a clean and safe environment for patients, adhering to infection control protocols. Qualifications Current Registered Nurse (RN) license in the state of Nevada. Knowledge of home health / hospice palliative medicine principles and Medicare guidelines preferred. Experience in hospital medicine, emergency room, long-term care, or memory care is a plus. Strong clinical skills in acute care settings, including proficiency in aseptic technique. Excellent communication skills, both verbal and written, with the ability to educate patients effectively. Ability to work independently while managing multiple priorities in a home health environment and able to integrate with the clinical team including therapist, social workers, nurse aides , Physicians and office staff.
