Key Responsibilities:

Deliver skilled, compassionate in-home nursing care Complete thorough patient assessments and document findings via OASIS accurately and promptly Provide wound care, medication administration, education, and disease monitoring Create individualized care plans in collaboration with physicians and interdisciplinary teams Ensure prompt, effective communication of any changes in patient condition Maintain accurate, timely records using HomeCare HomeBase (preferred)

Qualifications:

Current, active RN license At least one year of recent RN home health experience Proficiency with OASIS is required Familiarity with HomeCare HomeBase is strongly preferred Reliable transportation and a valid drivers license Confidence in independently managing a full patient caseload with daily visits
