Schedule & Assignment Details:

Full-time:

Monday through Friday On-call coverage may be required 13-week assignment with potential for extension Structured daily visit expectations

Key Responsibilities:

Deliver high-quality, in-home nursing care and skilled interventions Independently conduct thorough patient assessments and accurately complete OASIS documentation within required timelines Collaborate closely with physicians and interdisciplinary care teams to formulate and implement individualized patient care plans Perform wound care, manage medications, provide patient/family education, and monitor for health status changes Communicate promptly with the clinical team regarding patient updates, changes in condition, and care needs

Qualifications:

Current, active RN license At least 1 year of recent home health RN experience Proficiency with OASIS documentation is required HomeCare HomeBase experience strongly preferred Valid drivers license and reliable transportation for community-based visits Ability to balance a structured schedule with multiple patient visits per day
