Your next nursing opportunity starts here. We are seeking a committed Registered Nurse with strong OASIS expertise for a 13-week, full-time home health assignment. This position is designed for a confident clinician skilled in both patient care and meticulous documentation, ready to make a meaningful difference in the lives of patients in their own homes.

Schedule:

Full-time, Monday through Friday On-call duties may be required Structured daily visit expectations

Key Responsibilities:

Deliver skilled in-home nursing care and evidence-based interventions Perform comprehensive patient assessments and complete OASIS documentation accurately and promptly Coordinate with physicians and multidisciplinary teams to develop and implement personalized care plans Carry out wound care, medication administration, disease management, and patient/family education Monitor and communicate any changes in patient condition to the care team

Preferred Qualifications:

Active RN license (in good standing) Minimum of 1 year of recent home health RN experience Proficient in OASIS documentation Experience with HomeCare HomeBase is strongly preferred Proven ability to manage a full-time caseload and adhere to set schedules Valid drivers license and reliable transportation
