• Responsibilities
• + Provide skilled nursing services in patients' homes throughout the assigned coverage area + Manage patient caseloads and coordinate ongoing care + Complete comprehensive patient assessments and documentation + Develop, implement, and monitor individualized care plans + Collaborate with physicians and interdisciplinary care teams + Educate patients and caregivers on treatment plans and disease management + Ensure accurate and timely documentation of all patient care activities •Requirements•+ New York Registered Nurse (RN) license + Home Health experience required + Strong case management experience + Ability to work independently in the field + Reliable transportation for local travel + Strong assessment and documentation skills •Preferred Skills•+ Experience with HCHB (Homecare Homebase) documentation + Strong care coordination and case management skills + Excellent communication and organizational abilities + Experience managing a home-based patient population •Additional Information•+ •
