One week per month (Friday to Friday) Position Overview We are seeking a dedicated and compassionate Registered Nurse (RN) to provide high-quality home health care services throughout Erie County, NY . The ideal candidate will be responsible for delivering skilled nursing care, patient education, wound management, and IV therapy services in patients' homes while ensuring compliance with clinical standards and care plans. Key Responsibilities Provide comprehensive skilled nursing assessments and ongoing patient evaluations in the home setting. Perform advanced wound care management , including assessment, treatment, and documentation of wound healing progress. Administer and manage IV therapies , including medication administration and monitoring for complications. Educate patients, families, and caregivers regarding disease management, medications, treatment plans, and preventive care. Develop, implement, and update individualized patient care plans in collaboration with interdisciplinary healthcare teams. Maintain accurate and timely clinical documentation in accordance with agency, state, and regulatory requirements. Monitor patient conditions and communicate changes to physicians and care teams as appropriate. Ensure adherence to infection control, safety, and quality care standards. Participate in on-call rotation as required. Required Qualifications Active and unrestricted Registered Nurse (RN) License in the State of New York . Current BLS certification (if required by agency policy). Strong clinical assessment and critical thinking skills. Ability to work independently and manage a patient caseload effectively. Valid driver's license and reliable transportation. Preferred Qualifications Previous Home Health Nursing experience.

Strong experience in:

Wound Care IV Therapy/IV Management Patient and Caregiver Education Familiarity with home health documentation systems and regulatory requirements. Productivity Expectations Maintain a productivity standard of 30 points per week . Travel & Mileage Mileage reimbursement provided between patient visits (from first patient to last patient visit) .
