Summary:
Works collaboratively with the multidisciplinary health team, is responsible for providing comprehensive care to patients by assessing, planning, implementing and evaluating care for patients. Position focuses on inpatient/outpatient care, specifically care of the medical hematology and oncology patient as well as consultations and management of inpatient medical hematology & oncology patients. Participates in patient and family teaching. Provides leadership by working cooperatively with health care team members in maintaining standards for professional practice in the clinical setting. Supports delivery of evidence-based acute assessment and management. Develops and implements patient management plans, records, progress notes and assists in provision of continuity of care. Participates in performance improvement processes as appropriate. The Infusion Center Advanced Practice Provider (APP) functions as an independent health care provider who, working collaboratively within a multidisciplinary team, is responsible for coordinating comprehensive care in an outpatient setting through assessing, planning, implementing, and evaluation. He/She demonstrates a high degree of clinical expertise in working with patients with acute and chronic illnesses including oncology patients. He/She is responsible for assessment, treatment, management, education, health promotion, and care coordination for patient during treatments. The APP demonstrates an advanced level of medical oncology clinical knowledge, communication, interdisciplinary collaboration, and professional practice. He/She works with administration, nursing, and physicians to assure safe, effective, quality patient care and to promote Catholic Health's mission, vision and values. In addition, he/she has a strong understanding of the business end, including but not limited to oncology billing and coding.
Responsibilities:
EDUCATION
Bachelor's degree Graduate of an approved NCCPA (National Commission on Certification of Physician Assistants) and CAHEA (Committee on Allied Health Education and Accreditation of the AMA) accredited Physician Assistant training program; OR Graduate degree as Nurse Practitioner Maintains a current license in New York State as a nurse practitioner (Active APNP license) or Physician Assistant (Active PA license) BLS and ACLS certification Certification in oncology Certification in administration of chemotherapeutic agents and/or biologic agents
EXPERIENCE
Two (2) years of oncology experience preferred
KNOWLEDGE, SKILL AND ABILITY
Ability to manage acute sensitivity/allergic reactions Excellent verbal and written communication skills Professional demeanor Ability to work as a member of a multidisciplinary team Ability to prioritize tasks
